Superior epigastric artery perforator flap for reconstruction of deep sternal wound infection

چکیده

Background The treatment of deep wound sternal infection requires loco-regional pedicled flaps, usually with muscular flaps. Perforator propeller flaps represent the ultimate progress in history reconstructive surgery. We report here our experience superior epigastric artery perforator (SEAP) to repair defect. Patients and methods Six patients presenting wounds were treated SEAP flap, between March 2015 June 2017. mean age was 71.5 (range 53–83) years. length width defect 16.2 × 7 cm (ranging 8–20 4–10). An elliptical skin flap on harvested inframammary fold rotated up 90° cover Results All achieved a successful entire coverage size paddle 20.2 7.3 14–27 6–9). able provide complete cover. Venous congestion present five cases adequately by leech therapy; necrosis distal one case, interesting superficial two but tissues remaining viable mediastinum: an infected required revision. Satisfyingly, at 2-years postoperative follow-up all-patients alive mediastinal Conclusions is alternative muscle achieve large infection.

Porcine Model for Deep Superior Epigastric Artery Perforator Flap Harvesting: Anatomy and Technique

1 BACKGROUND Microsurgical training on rats before starting with clinical practice is a well-established routine. Animal model training is less widespread for perforator flaps, although these flaps represent a technical challenge. Unlike other flaps, they require specific technical skills that need to be adequately trained on a living model1: a cadaver is not enough because no bleeding, vessel ...

The deep inferior epigastric artery perforator flap for autologous reconstruction of large partial mastectomy defects.

BACKGROUND Breast conservation surgery in the treatment of early stage breast cancer has become increasingly utilized as a means to avoiding mastectomy. While partial mastectomy defects (PMDs) may often be cosmetically acceptable, some cases warrant consideration of reconstructive options, and while several reconstructive options have been described in this role, a series of deep inferior epiga...

A Preoperative Marking Template for Deep Inferior Epigastric Artery Perforator Flap Perforators in Breast Reconstruction

Preoperative perforator marking for deep inferior epigastric artery perforator flaps is vital to the success of the procedure in breast reconstruction. Advances in imaging have facilitated accurate identification and preselection of potentially useful perforators. However, the reported imaging accuracy may be lost when preoperatively marking the patient, due to 'mapping errors', as this relies ...

Lymphatic Drainage of the Deep Inferior Epigastric Artery Perforator Flap after Breast Reconstruction

Background: Transplanted tissue in breast reconstruction is routinely performed without any lymphatic anastomosis. Edemas might be observed at the level of these transplanted tissues. New lymphatic routes are supposed to develop but little is known about the development of new lymphatic vessels in the transplanted tissue after free flap transfer. The aim of the study was to evaluate the lymphat...
